Student health record information is required to be on file in the school office for all students. A current exam form(s) is/are required at the beginning of a school year for the following grade levels.

Preschool - Physical Exam
Kindergarten - Physical Exam, Dental Exam and Eye Exam
Second Grade - Dental Exam
Sixth Grade - Physical Exam and Dental Exam
New Student (Grades 1-8) - Physical Exam, Dental Exam and Eye Exam
